Request

Step 1

Fill out the ONLINE REQUEST FORM.

(Requests sent via e-mail will NOT be processed.)

Step 2

After successfully submitting the request form, it will be queued for checking.

If there are no concerns, kindly wait three (3) working days for your PAYMENT ADVICE email to be sent by businessoffice@ndgm63.edu.ph 

Check your Spam or Important folders in case the email gets randomly saved there.

Step 3

Once you receive the Payment Advice, please proceed with your payment. Make sure to take a clear photo or screenshot of the successful transaction receipt.

Refer to the Payment Guide below for more info.

Step 4

Open your Payment Advice email and click on the link to "Send Proof of Payment". Fill out the form and upload your transaction receipt. 

Step 5

Please wait for your payment to be verified. After verification, your request will now be processed for release.
